Physical - 60 minutes - $250

Adult preventive physical (includes lab panel: CBC, BMP, A1c, cholesterol panel) 

Pre-operative physical (includes CBC, BMP, A1c)  

Second Opinions: review of tests, labs, imaging, or consults from another clinic 

NO chronic disease management, medication refills or controlled substances will be provided.

Your appointment fee may be applied for credit towards a membership at time of appointment
